A case of 41 years old male, HIV infection, presented with left cheek huge mass measuring 5*5 cm2 in size. Biopsy over left cheek revealed plasmablastic lymphoma.

It is now considered a distinct subtype of diffuse large B cell lymphoma, seeing more commonly in patients with HIV infection. Because of its distinct clinical and pathological features, such as lack of expression of CD20, plasmablastic morphology, and clinical course characterized by early relapses and subsequent chemotherapy resistance, PBL can represent at diagnostic and therapeutic challenge for pathologists and clinicians alike.
